Enrollment for 2009-2010 season begins in October. Necessary forms will be uploaded to the website after November 15, 2009 at which time they can be printed and filled out. *Please note organizational leaders received re-enrollment information in October for club members. Application - adults wanting to be leaders can fill
out this form and return it to the Extension office.
Reference Form - Adults must have 3 friends, co-workers, etc. fill out this
form and return to the Extension office before being approved as a leader. **You must also complete a
