Forms to Print and Sign for ADULT VOLUNTEERS - Give to your club leader after enolling electronically.

Adult Code of Conduct - one for each leader enrolling in 4-H. Can be found online at: http://www.ext.colostate.edu/ye/family/forms/permission_adult.pdf

Enrollment Form - one for EACH leader enrolling in 4-H. To print this, you log in to 4-H Online. Once you have updated all of your information, you need to return to the family screen (where all members of your family are listed.) Under the Member/Volunteer List, you will see a section titled "Member Reports." Select the leader's name from the list. Then in the drop down menu under it select, "Member-Enrollment Form." Print and sign the PDF and give it to your club leader.

HINT: The form will be a pdf document will be a "pop-up." If your pop-up blocker blocks the download, click on the pop-up blocker bar at the top of your screen and select "download," then repeat the steps of selecting the leader's name from the list and then selecting "Member-Enrollment Form."

NOTE: We are aware the the numbers on the printed form may not match what you entered. The state is working on the problem. Just fix the numbers by hand for right now.

Members participating in animal projects must also complete three additional forms:

Code of Show Ring Ethics Form - For all animal projects

Animal Care and Housing Form - For all animal projects, only complete the first page if the animal is housed at your home

USDA Wholesome Meat Act Agreement Form - All Rabbit, Poultry, Goats, Swine, Sheep and Beef projects fill out this form because each of these animals could potentially enter the food system
